The burden of human disease related to medically important fungal pathogens is substantial. An improved understanding of antifungal pharmacology and antifungal pharmacokineticspharmacodynamics has resulted in therapeutic drug monitoring (TDM) becoming a valuable adjunct to the routine administration of some antifungal agents. TDM may increase the probability of a successful outcome, prevent drug-related toxicity and potentially prevent the emergence of antifungal drug resistance. Much of the evidence that supports TDM is circumstantial. This document reviews the available literature and provides a series of recommendations for TDM of antifungal agents.

Teratosphaeria suttonii (=Kirramyces epicoccoides) is a leaf pathogen that can cause premature defoliation, reduced growth and vigor, and subsequent tree death of many Eucalyptus species. Although the fungus primarily infects mature leaves in the lower canopy, infections can spread to younger leaves during continued epidemics or when trees are stressed. Teratosphaeria suttonii has a wide distribution in Australia and has been introduced to many other parts of the world, most probably with germplasm used to establish plantations. The aim of this study was to establish the phylogenetic relationships between T. suttonii isolates from different countries and to consider whether cryptic species exist in a species complex. DNA from parts of the nuclear ribosomal internal transcribed spacer, β-tubulin, and elongation factor-1α genes was sequenced and analyzed for isolates from throughout the range of T. suttonii in Australia, and from six countries (China, Indonesia, South Africa, Uruguay, United States, and Vietnam) where the pathogen is introduced. Morphometrics of conidia produced both in vivo and in vitro were also considered. Analysis of the sequence data resulted in incongruent genealogies. Furthermore, groups of isolates in the genealogies could not be linked to area of origin. Similarly, differences in conidial morphology could not be linked to any of the phylogenetic groups. There was no evidence of distinct species boundaries, and isolates from Australia were closely related to those from other parts of the world. The results of this study support the treatment of T. suttonii as a morphologically and genetically diverse species in its natural range in Australia. The diversity is reflected in introduced populations.

The purpose of the Death Verification Service is to elucidate the causes of deaths that occur without medical assistance and of ill-defined deaths. In recent decades, the epidemiological reality of fungal infections has changed due to the rise in opportunistic infections chiefly in immunocompromised patients.A study of fungal diseases in autopsies performed in the Death Verification Service of the Medicine School in S?o José do Rio Preto between January 2000 and December 2009 was made.Sixty-seven cases of fungal disease, most involving men (70%), were found in 4824 autopsies. Cryptococcosis was the most prevalent (45%), followed by paracoccidioidomycosis, candidiasis, histoplasmosis, aspergillosis and mucormycosis. Associations between AIDS (. n=. 14) and fungal diseases were identified for cryptococcosis (36%), candidiasis (28.5%) and histoplasmosis (28.5%). Pneumonia, AIDS and fungal diseases were evident in 26% of the cases, with the most prevalent etiologies being . Cryptococcus neoformans (55.5%) and . Histoplasma capsulatum (22%). Pneumonia alone occurred in 43% of cases, with cryptococcosis (53%) and paracoccidioidomycosis (33%) being the main infectious agents. Diabetes mellitus was associated with candidiasis in two cases and aspergillosis in one. One case of renal transplantation linked to paracoccidioidomycosis and one case of bone marrow aplasia with mucormycosis were reported.Despite the reduction in the number of autopsies over recent decades, these findings suggest that this procedure is useful to provide additional data on the etiology, underlying disease and specific risk factors, essential for quality control and to improve treatment protocols.

Endophytic fungi are widespread in plants and affect the host fitness and population size. We found 12 fungal taxa in C. secalina, a critically endangered species in several European countries, at two study sites in Poland. The most frequently occurring fungal taxa were Colletotrichum destructivum and Acremonium sp. Both taxa were found in half of the examined tussocks. The highest number of fungal taxa was noted in the C. secalina plants growing in the roadside area, where 7 of the 12 identified fungal taxa occurred. These fungi, inhabiting leaf tissues, are known for their pathogenicity but no visible symptoms of any diseases were observed on C. secalina leaves. This suggests that these fungi are latent pathogens.

Alzheimer's disease is a progressive neurodegenerative disorder that leads to dementia mainly among the elderly. This disease is characterized by the presence in the brain of amyloid plaques and neurofibrillary tangles that provoke neuronal cell death, vascular dysfunction, and inflammatory processes. In the present work, we have analyzed the existence of fungal infection in Alzheimer's disease patients. A proteomic analysis provides compelling evidence for the existence of fungal proteins in brain samples from Alzheimer's disease patients. Furthermore, PCR analysis reveals a variety of fungal species in these samples, dependent on the patient and the tissue tested. DNA sequencing demonstrated that several fungal species can be found in brain samples. Together, these results show that fungal macromolecules can be detected in brain from Alzheimer's disease patients. To our knowledge these findings represent the first evidence that fungal infection is detectable in brain samples from Alzheimer's disease patients. The possibility that this may represent a risk factor or may contribute to the etiological cause of Alzheimer's disease is discussed.

Ophidiomyces ophiodiicola, the causative agent of snake fungal disease, is a serious emerging fungal pathogen of North American-endemic and captive snakes. We provide a detailed literature review, introduce new ecological and biological information and consider aspects of O. ophiodiicola that need further investigation. The current biological evidence suggests that this fungus can persist as an environmental saprobe in soil, as well as colonizing living hosts. Not unlike other emerging fungal pathogens, many fundamental questions such as the origin of O. ophiodiicola, mode of transmission, environmental influences, and effective treatment options still need to be investigated.
